AVIATION ROOT CAUSE ANALYSIS (RCA)
COURSE INTRODUCTION
Repetitive audit and investigation findings are most often the result of misidentifying “symptoms” as the underlying “problem(s)” in need of correction. This misidentification leads directly to unwanted reoccurrences that escalate in both frequency and severity as the underlying problem(s) remain unabated. Further, it is critical that the entire organization looks at operational risk in the same way from the perspective of probability and severity. This course provides the attendee with an overall understanding of the relationship between underlying causations and the multiple symptoms that they routinely produce as well as a proven methodology for assigning risk levels.
